GS-3: Science and Technology—Developments and their Applications and Effects in Everyday Life; Awareness in the fields of Biotechnology.

Context: The Union Health Ministry has issued an advisory to regulate stem cell therapy, reiterating that only approved indications may be offered as standard clinical care and restricting unproven interventions.

Key Highlights of the Advisory

• Approved indications: Stem cell therapy may be offered as standard clinical care only for Ministry-approved indications, covering 18 adult and 14 paediatric conditions. 

• Only appropriately performed haematopoietic stem cell transplantation (HSCT) is recommended for these indications; mesenchymal stem-cell transplantation is not recommended for any disease. 

• Autism Spectrum Disorder: Use of any type of stem cell for ASD is restricted to duly approved clinical trials and cannot be offered as routine, standard or commercial clinical care. 

• Implementation: States/UTs that have adopted the Clinical Establishments (Registration and Regulation) Act, 2010 have been directed to ensure compliance by State/District Regulatory Authorities and concerned government and private clinical establishments. 

• Professional accountability: Unauthorised administration, prescription, promotion or advertisement of stem cell therapy beyond approved indications constitutes professional misconduct; State Medical Councils may take disciplinary action following due process.

Understanding Stem Cell Therapy

• Stem Cell Therapy: A therapeutic approach that uses stem cells to replace, repair or regenerate damaged or diseased cells and tissues. It forms an important component of the broader field of regenerative medicine. 

• Stem Cells: Undifferentiated cells characterised by two key properties: 

• Self-renewal: Ability to produce more stem cells through cell division. 
• Differentiation: Ability to develop into specialised cell types, with the range of differentiation depending on the type and potency of the stem cell. 

• Major Types of Stem Cells: 

• Embryonic Stem Cells (ESCs): Pluripotent cells derived from the inner cell mass of an early-stage embryo, capable of differentiating into many specialised cell types. 
• Adult/Somatic Stem Cells: Found in developed tissues and generally have a more limited differentiation potential; e.g., haematopoietic stem cells can give rise to different blood-cell lineages. 
• Induced Pluripotent Stem Cells (iPSCs): Mature, differentiated cells that are genetically reprogrammed to acquire pluripotent characteristics. 

• Therapeutic Potential: Stem cells have potential applications in tissue repair and regeneration and in treating certain diseases. However, their therapeutic potential does not automatically establish a treatment as clinically proven.

Regulatory Framework for Stem Cell Research and Therapy in India

• National Guidelines for Stem Cell Research, 2017: Issued jointly by the ICMR and Department of Biotechnology (DBT), these guidelines provide the framework for stem cell research and its clinical translation. 

• Clinical Trials: Stem cell therapies that are investigational must undergo the prescribed clinical-trial and regulatory approval processes rather than being offered as established treatment. 

• Regulatory strengthening: The Drugs Rules, 1945 were amended in 2026 to bring cell or stem-cell-derived products, along with gene therapeutic products and xenografts, under the Central Licences Approving Authority (CLAA) framework. 

• Professional regulation: The NMC's September 2026 advisory reinforces the approved-indication requirement and directs State Medical Councils to examine alleged violations and take appropriate disciplinary action where professional misconduct is established following due process. 

Safeguards and Significance of the Advisory

• Patient safety: Restricting unproven interventions helps protect patients from therapies whose safety and efficacy have not been adequately established. 

• Evidence-based medicine: The advisory reinforces the distinction between approved clinical applications and investigational therapies requiring clinical-trial evaluation. 

• Protection from misleading claims: Restrictions on unauthorised promotion and commercial provision can help prevent unsupported therapeutic claims from influencing treatment decisions. 

• Regulatory accountability: The advisory strengthens implementation through State/District Regulatory Authorities, clinical establishments and State Medical Councils.
